Provide aged care provider with a copy of clients “Enduring Power of Attorney” and or “Enduring Guardianship” appointment (As applicable)
Provide aged care provider a copy of clients “Aged Care Client Record” (ACCR) at the appropriate time, for their review, information and reference.
Informing a client immediately when and where vacancies become available.
In understanding the fee structures of the aged care provider.
The completion of all documentation required by the aged care facility and Government.
Providing post placement assistance during the settling in period and throughout the resident’s time in care.
Provide aged care provider with a copy of clients “Combined Assets and Income Assessment” letter issued by Services Australia or Department of Veterans Affairs.
General negotiation with aged care providers.
Arrange visits of the selected care aged care
Producing aged care provider information such as “Resident/Accommodation Agreement”, Village Rules etc.
The formal introduction of a client to an aged care facility.
Provide clients with application form; Permanent Residential Aged Care – “Request for a Combined Assets and Income Assessment” (Form SA 457) and assist in its preparation for submission to Services Australia or Department of Veterans Affairs
