We Simplify Aged Care

As part of our vast and highly specialised services we will assist clients in finding an appropriate aged care facility in their preferred location, assisting in the completion of all documentation, understanding the fee structures of an aged care provider, negotiating the Refundable Accommodation Deposit (RAD) where applicable, arranging facility visits and producing aged care provider information, such as “Respite Care Agreement”, “Resident/Accommodation Agreement” and “ and Village Rules etc.

 Because each person is different and everyone’s circumstances and reasons for needing to move into an aged care facility are unique we will provide you a tailored aged care solution to meet your lifestyle and financial needs.

Our comprehensive service includes:

  • Client consultation in the comfort and privacy of their own home and at a time convenient to them (Service available 7 days a week and outside normal business hours for the convenience of client’s and their families. Family members are encouraged to attend where possible).
  • Awareness of current care options
  • Understanding residential aged care
  • Assist with all steps of the application for care process
  • Informing a client immediately when and where vacancies become available as we receive the latest information, either directly from residential aged care providers themselves, or via the Aged Care Assessment Team (ACAT) for permanent placement, or the Commonwealth Respite Care booking office, for the purposes of respite care placement.
  • Advocating on a client’s behalf at their preferred aged care home
  • Providing post placement assistance during the settling in period and throughout the resident’s time in care.
  • Provide clients with application form; Permanent Residential Aged Care – “Request for a Combined Assets and Income Assessment” (Form SA 457) and assist in its preparation for submission to the Department of Human Services or Department of Veterans Affairs 
  • Researching potential aged care facilities within a client’s preferred location.
  • Accompany clients and their families where applicable, on inspection tour of pre – selected care facilities in conjunction with care facility staff.
  • The formal introduction of a client to an aged care facility.
  • Negotiating Refundable Accommodation Deposit (RAD) with care provider (As applicable).
  • General negotiation with aged care providers.
  • Provide aged care provider a copy of clients “Aged Care Client Record” (ACCR) at the appropriate time, for their review, information and reference.
  • Provide aged care provider with a copy of clients “Combined Assets and Income Assessment” letter issued by the Department of Human Services or Department of Veterans Affairs.
  • Provide aged care provider with a copy of clients “Enduring Power of Attorney” and or “Enduring Guardianship” appointment (As applicable)
